Insurance Plans

We are an in-network provider for the insurance plans below. We do not accept Medicare, Medicaid, County Care, or most HMO plans.

Insurance Quotes & Claims

We do our very best to help you understand your insurance benefits, but please keep in mind that coverage quotes and cost estimates are not a guarantee of payment.

To make things easy for you, we will bill your insurance provider directly after each session. You will be responsible for your deductible, copay, and/or coinsurance amounts as specified by your plan.

Because insurance companies make their final coverage decisions only after they receive and process (adjudicate) each clinical claim, any balance or fees not covered by your plan will ultimately be your responsibility.

Out-of-Network Reimbursement

If you have an insurance plan that we do not accept, you may be able to access care with us using any out-of-network benefits. After you make your payment for services, we will provide you with a monthly "superbill" (an itemized receipt) that you can submit to your insurance provider for potential reimbursement.

We strongly encourage you to contact your insurance company before your first visit to understand how your specific out-of-network benefits work. Please note that seeking reimbursement is a direct matter between you and your insurance provider and does not involve Leap Therapy Chicago.

Fees & Payments

The following are fees for our most common services:

• Diagnostic Evaluation (first session or re-evaluation): $220

• 55-Minute Therapy Session: $180

Payments & Billing

We accept all major credit and debit cards, HSA and FSA cards, checks, and cash. To make billing seamless, we require all clients to keep a secure credit or debit card on file. Your card on file will be automatically charged after each appointment or once your insurance claim has been fully processed, unless you request an alternative payment method beforehand. Please note that if an insurance claim is denied, you remain responsible for the full session fee. 

Sliding Scale Appointments

Each of our therapists reserves a limited number of appointment slots specifically for clients experiencing financial hardship. If you do not have insurance and cannot afford our standard fee, you may request our "Application for a Reduced Rate." We utilize a sliding scale based on income, family size, and federal guidelines to determine eligibility and reduction amount. Please keep in mind that these slots are limited and qualification is not guaranteed.

Good Faith Estimate

If you do not have insurance or choose not to use your insurance for scheduled, non-emergency care, federal law protects you from unexpected costs. Under the No Surprises Act, you have the right to request a Good Faith Estimate outlining your expected charges at least one business day before your scheduled appointment. Cancelation Policy

Your therapy time is reserved exclusively for you. If you need to cancel or reschedule, please provide us with at least 24 hours' notice. Cancelations made with less than 24 hours' notice, as well as missed appointments (no-shows), will incur a standard $75 fee. Please note that insurance providers do not cover costs for canceled or missed sessions.
